Be prepared

The form will take about 5–7 minutes to complete. Once you have started your request you will not be able to save your progress. Please be sure you have enough time to complete the form before you begin.

You will also need to verify your identity, and if you’re requesting documents on behalf of an adult (age 18 or older) or independent student (16 or 17 and declared themselves independent), provide proof of consent or legal guardianship.
